What documents are required for name correction in a birth certificate?

For those who’ve gone through a name correction process in their birth certificate, what documents did you have to submit? I’ve heard it usually needs an affidavit, school records, and ID proof, but I’m not sure if all states ask for the same.

How to correct your name on the birth certificate and papers required

    Shows the wrong spelling that has to be fixed.

  2. Notarised affidavit
    A simple sworn statement on stamp paper saying the new spelling is correct.

  3. Aadhaar copy of parent or child
    Proves who is applying and links the correct name to an official ID.

  4. Early-life proof of correct name
    Any one paper dated before the child turned seven:

    • Hospital discharge sheet
    • Immunisation card
    • First school admission form
    • Parent’s marriage certificate
      These show the intended spelling from the start.
  5. Fee payment proof
    Screenshot or PDF of the online payment (usually ₹50–₹200).

If the child is above one year or you want a completely new first name, most states also ask for a court order first. Finish that step, then upload these five items.
