How to fill and submit the ICICI Bank transaction dispute form (TDF)?

I had a problem with a transaction on my ICICI Bank account and found out I need to submit the Transaction Dispute Form (TDF). Has anyone here filled and submitted this form before? What steps did you follow?

I was also curious about this when I first spotted a wrong debit on my ICICI Bank card last month and wanted my money back.

How to Fill and Submit ICICI Bank Transaction Dispute Form (TDF)

  • Download the Customer Dispute Form (CDF) from icicibank.com > Forms > Transaction Dispute Form or collect it from any ICICI branch

  • Write your 16-digit card number (only last 4 digits on paper for safety), name, mobile, e-mail, statement date, transaction date, exact amount and tick the reason (unauthorised, goods not received, double debit, wrong amount, cancelled subscription)

  • Attach proof pictures: SMS or e-mail alert that shows the debit, ATM slip (if cash not dispensed), merchant bill (if amount differs), cancellation e-mail, delivery note (if goods not received), copy of the card (front side, last 4 digits visible) to prove the card was in your possession, location proof (HR letter, hotel stay invoice, post-paid mobile bill, or statement of any other bank card used on the same date) to show you were not at the place of transaction, and a cancelled cheque or first page of passbook so the bank can credit the money back if your claim is upheld

  • For online submission, log in to ICICI Internet Banking or iMobile Pay app, go to Service Request, Card, Transaction Dispute, upload the filled PDF and proof, click submit, and you get an instant ticket number, save it

  • For e-mail submission, sign the form, scan it with all proofs and send to [email protected] with the ticket number in the subject line

  • Use the ticket number in the iMobile app or call 1800 2662 to check status; if they ask for more papers, upload the same day to keep the file inside the 45-day lane.

  • Keep every proof ready before you start; uploading everything together keeps your dispute inside the 45-day lane and avoids extra back-and-forth.
