I’m planning to file a claim for my pet’s treatment and want to be prepared. Has anyone’s pet insurance in India asked for specific documents and which ones did you need to submit to complete the claim?
Many insurers in India ask for the same set of papers, if you have them ready, your claim is usually cleared in 10–15 working days.
Documents You Need to Submit for a Pet Insurance Claim
- Claim Form: this is the basic form where you fill in your details, your pet’s details, and a short description of what happened. It must also be signed by the vet who treated your pet.
- Original Vet Bills & Payment Receipts: these are the printed bills from the clinic that show the date, name of the treatment, medicines used, and the exact amount you paid. Always ask for a proper bill with the clinic stamp.
- Vet Prescription & Medical Records: the vet’s handwritten or computer print-out that explains why the treatment was needed. It can be the case sheet, discharge summary, or daily treatment chart.
- Diagnostic / Lab Reports: if your pet had a blood test, x-ray, ultrasound, or any other test, attach the printed results. These reports help the insurer see that the treatment was necessary.
- Vaccination Card Copy: a photocopy or clear photo of the card that shows all vaccinations are up-to-date. Most insurers insist on this to prove the pet was healthy before the illness.
- Photo ID of Pet: clear colour photos of your pet from the front and side so the company can confirm it is the same animal covered under the policy
- Policy Copy / E-Card: a print-out or PDF of your policy certificate that shows the policy number, start date, and coverage amount.
- Bank Details: a cancelled cheque or the first page of your passbook so the insurer can transfer the claim money directly to your account.
- Death Certificate & Post-mortem: needed only if you are claiming for the pet’s death. The certificate should be issued by a qualified vet and, in some cases, a post-mortem report is also asked for.
- Police FIR / Newspaper Ad: needed only if your pet is lost or stolen. You have to file an FIR at the local police station and, in some policies, also give a copy of the newspaper advertisement.
