What documents are required to raise a transaction dispute with ICICI Bank?

I need to raise a dispute for a wrong transaction in my ICICI Bank account, but I’m not sure what paperwork is needed. Has anyone here filed a dispute with ICICI? What documents did they ask for and did you have to provide anything extra before the claim was accepted?

I was also worried about this when I saw a wrong debit on my ICICI Bank card last month and wanted my money back.

Documents You Need to Raise a Transaction Dispute with ICICI Bank

ICICI Bank accepts disputes online (fastest) or by e-mail, both routes ask for the same set of papers. Keep them ready and you can file the complaint in under five minutes.

  • Filled Customer Dispute Form (CDF) : download from icicibank.com then go to the Forms then Transaction Dispute Form, write your card number, transaction date, amount and tick the correct reason (unauthorised, goods not received, double debit, etc.).

  • Proof that the charge happened : SMS alert, e-mail statement, or app screenshot that shows the debit you are fighting.

  • Proof that the charge was wrong : ATM slip if cash not dispensed, merchant bill if amount differs, cancellation e-mail, delivery note if goods not received, or any message you sent to the merchant asking for a refund.

  • Copy of the card (front side) : only last 4 digits should be visible; this proves the card was in your possession

  • Photo ID proof : PAN or Aadhaar copy (only if you send paper form, online login already verifies you).

  • Location proof (for fraud cases) : HR letter, hotel stay invoice, post-paid mobile bill, or statement of any other bank card used on the same date, this shows you were not at the place of transaction.

  • Bank details : cancelled cheque or first page of passbook so the bank can credit the money back if your claim is upheld.

  • Police complaint copy (optional but helpful) : if you have filed an FIR or cyber-crime report, attach it, not compulsory for the first claim.
